What is Home Health Care? 

Home Health Care refers to the medical care and services that are provided by a licensed and certified home health agency  to individuals who are home bound. The need for these services typically are required after a surgery or during an illness.   All services are performed under a physician's order in the privacy of the individuals’ home .

Home health services include the following:

  • Nursing

    • Our trained skilled staff provides care which may include: Behavioral Health, Chronic Disease Management, Teaching and Training, Medication Management, Fall Prevention, Post-Operative care, Wound care and Much More​

  • Physical Therapy

    • provides assistance to an individual by increasing mobility, strength and flexibility.  The goal of our physical therapist is to restore optimum functionality.

  • Occupational Therapy

    • shows and encourages patients to use adaptive equipment, such as braces, wheelchairs, and eating aids allowing them to function more independently

  • Speech Therapy

    • helps patients by assessing and treating speech disorders - including language, communication, cognitive and swallowing disorders​

Other specialized therapies are also available to assist, encourage and support an individual’s quality of life.

To qualify for Home Health Care you will need to have doctor’s orders, or a referral, to start care. Once we receive your referral, we schedule an appointment to come to your home for an initial evaluation to ask questions regarding your health and ensure we meet your needs. Once we begin your care, we will be in contact with your doctor regarding your care and keep your doctor updated on your progress.

 

Our goal is to help you heal, maximize your self-sufficiency and independence, improve or maintain your current condition, and slow the decline of any health conditions. We strive to give our patients a dignified and high quality of life.

Who Qualifies for Home Health Care? 

In order to qualify for Home Health Care you must have a medical need, be endorsed by your physician and be considered home bound. Being home bound means that it takes great effort for you to leave your home in order to receive the services required by your referring physician (hospitalist or Primary Care Provider).

What Insurances Do We Accept?

We are fully Medicare and Medicaid certified

We are contracted with insurance plans from:  Western Sky/Allwell, Humana, Molina, Presbyterian, Cigna, and Blue Cross Blue Shield.
